Work Permit Information: |
| Students age 14 - 15 can apply for a work permit at the Superintendent's Office located at Jay Middle School. The entrance the the superintendent's office is in the back of the high school / middle school. |
- The student must have a job already lined up before they can apply for a work permit.
- The student must be accompanied by parent or legal guardian. (Work permit forms will not be mailed home for parent/guardian signature.)
- The student will need to bring their birth certificate and know their Social Security number.
- The student will need to provide the employer name, address & phone number, industry of the employer (type of business), plus the occupation of the student (a description of job duties).
- Once the permit is completed it will be mailed to the Department of Labor for approval.
- Once it is approved, they will mail it to the Superintendent's Office for a copy to be kept at Central Office. The remaining copy will be mailed to the student to take to their employer, this process generally takes 5-10 business days.

Children must be age 5 for kindergarten and 4 for pre-kindergarten registration on or before Oct. 15. It is necessary to have the child's original birth certificate, current immunization record and social security number at the time of registration.
